Do you happen to have any information on what kind of doctors to see for treatment of declining concentration/memory/overall cognitive ability due to long-term insomnia?

I have had insomnia for 17 years. My sleep right now is not nearly as bad as it used to be, but my ability to focus, think, communicate...has deteriorated to a debilitating degree. I have seen family doctors, a neurologist, psychologists and psychiatrists, so far with little progress and little pointer as to how to proceed further to remedy the problem. Any information/pointer that I could explore would be most appreciated.
