Questions: Does she eventually get deep restful sleep, or does it affect her wakefulness during the day?
How long has this been going on?
Has she seen a neurologist who specializes in sleep disorders?
I have the same symptoms, they have increased over the years to the point now that I startle, jerk, and scream, not just when falling asleep, but through the night, and now during naps.
I would recommend reading about various sleep disorders, ie., narcolepsy, REM sleep disorder, sleep apnea, and see if there are other symptoms that you have overlooked but when put together may help you decide if you should see a neurologist. There are many links on Sleepnet that are helpful. It's frustrating, so I feel for you guys. It's really frustrating for my family. Mine do affect my daytime awakeness and I am being treated for narcolepsy. For a while Klonipin (clonazepam) worked wonders at night. I never knew anyone could sleep through the night without jerking before that. But I built up a quick tolerance to it, so I am back to being a "jerk". I hope you find the answers you need, but go to the best sources possible.
