I use a flexible plastic mouth guard that slips over my bottom teeth each night. It was cast at the dentist's office from an impression of my own teeth, so the fit is good. I soak it 2 times a week in efferdent. they also make hard guards if you chew up the flexible one. I've been wearing the guard for over 2 years, at least a year longer than I've been on Bi-Pap. I'm not certain why I have bruxism (teeth grinding), but it is possible that I was grinding due to the stress of my heart and lungs having to work harder because of apnea episodes. there is no proof there, but it is not uncommon for ap-nee-acks to grind teeth.
