You can push and get a bipap or similar device, you just can't get there in one step. With most insurance companies you have to work your way up from a CPAP. Basic steps are:

1) Get CPAP and wait a short period of time
2) Complain of trouble, e.g., hard exhaling at high pressure (true in my case)
3) Get Doctor to whine to Insurance company and specifically say a BiPap or Smart Cpap is medically necessary and normal CPAP doesn't work.

It took me about a week or two before I got my BiPap. Unfortunately, this sort of patience will seldom get you a heated humidifier since most insurance companies will refuse. $100 vs $3-400. SOBs.
