CPAP, ORAL APPLIANCE... STILL HAVING APNEAS...HELP!!

I was diagnosed with severe OSA (88 apneas per hour average) in March '97. Used CPAP, Oral appliance alone and oral appliance with CPAP and had 1 night being tested on BiPAP to no avail...was still exhausted and falling asleep at inappropriate times. I had another sleep study last night (Sunday) and was told they still can't find a satisfactory pressure. The throat surgery is not covered by Ontario's medical insurance and would cost me over $1,000, which I can't afford, but I hear it's not too effective anyway. The only other suggestion made was a tracheotomy, but I don't want to go to that extreme until all else is exhausted. Has anyone else had this much difficulty or know of other options. I am a bit overweight (12lbs) but wasn't until 3 or 4 months ago and I'm beginning to lose again. I am at wits end. Thanks in advance for any suggestions.
