The fact that the REM periods happened in the last two naps points to narcolepsy. The sleep latency points to narcolepsy. However, I wouldn't make the diagnosis of narcolepsy just on the information provided in a MSLT. It is good if you are sitting on a fence and don't know which way to go. But the best indicators are the patient experiencing cataplexy. If you have documented cataplexy and EDS you don't really have to do a sleep study. But if you just have EDS and no other symptoms of Narcolepsy then you have to dig futher, do the sleep study and if normal then as a last resort do a MSLT and if you are leaning for narcolepsy and have the results you have here then you will probably be correct making the diagnosis of narcolepsy most of the time.
Remember many people with UARS were diagnosed with narcolepsy and idiopathic CNS hypersomnia.
So there could be an underlying cause that the sleep recording is not picking up that may be the culprit.
