It seems that there was a mistake in the titration of this pt. When after cpap options were exauhsted, epap was started @ too low of a pressure. A experianced technologist should know that epap should start @ the point where apneas were controlled. 8cm is the point where the apneas were controlled, for a while @ least.
Bipap should have started @ 11/8. If epap is started @ a pressure @ which point there were apneas, there a greater chance of the airway closing @ the pt. will more than likely not tolerate the change to bi-level. Given that this pt. did fine @ 8cm for an hour, it seems that the pt. tolerated pap therapy. Pt. should be brought back for another titration and performed by an experianced technologist. Thank You!
