Yes, certainly a better history is needed -- but
for now the problem is dDx in general.
dDx includes:
PLMD
UARS
OSA/H
Sleep-related GE reflux
Sleep-related laryngospasm (includes stridor)
Sleep-choking (generally considered psychogenic)
sleep-related asthma
fibromyalgia/chronic pain -> restlessness, fatigue
depression/anxiety -> awakenings
also think about drug effects, alcoholW/U on this one might include PSG with
Pes & pH probe, urine drug/EtoH screen,
psychometrics, careful upper airway exam
(fiberoptic scope) and PFTs at some point.
