the workup looks pretty good; I would be
looking at idiopathic hypersomnia as the
diagnosis. (he hasn't got a protozoan sleeping
sickness does he?)
treatment to include: good sleep hygiene, safety
counceling, trial of stimulants--> may start with Cylert,
then methylphenidate. Probably should manage his
mild OSA (there may be some additional non-apneic
snore arousals in there...)
Better keep close followup initially until
stabilized.Thanks for the case -- what is your approach?
