Bruxism

I have a 40 year old patient , slim, Mallanpati 2, with EDS. FAther had parkinsons and "leg jerking" Brother has "leg jerking". She had PSG with mild OSA , and evidence for PLM while awake.(RLS, too)but not real sleep onset insomnia). She had PLMD (while asleep) but PLMD arousal Index was marginally elevated. On close exam most other fragmentary arousals (many) are initiated by a Bruxism which is then associated with a leg jerk. (not counted in the PLMD arousal Index) She is on Prozac for "fibromyalgia" but no Alpha delta sleep. She does not have akathisia. She had a mouthguard but her problem is sleepiness.She is an accountant and doesn't like being drowsy. MSLT helped rule out narcolepsy. History of nightmares but manageable.

What would you do?

I will give follow up on what I did ... Especially if good advice is given and if it works!
