The baseline hypoxemia is either V/Q mismatch or hypoventilation. Cardiopulmonary work-up including ABGs are indicated with O2% below 92%. This baseline Sat will not go away with treatment of OSA if present. Causes for hypoventilation other than COPD include muscle weakness, hypoventilation syndromes, diaphragm paralysis, and hypthyroidism. Sleep Hypoventilation Syndrome is currently reserved for those patients who have hypoxemia after upper airway obstruction is corrected. Treat the underlying problem for daytime hypoxemia. Treat OSA and reassess. If persists in sleep after CPAP could then use bilevel. I use respiratory stimulants (acetazolamide, Provera) in such cases when hypoventilation persists during wakefulness.
