The history is consistent with OSA so at the outset there may be a false negative PSG 1 in 200-300 cases, so I would reassess hisotry and the circumstances of the PSG.More details on the PSG might help as in drug or alcohol withdrawal there might be increased REM. The other causes to consider for sleepiness in a 52 year old man are insufficient sleep, so a sleep diary is needed. The issues of drowsy driving need to be addressed.
