Sleep Hypoventilation and End Tidal CO2

1) What is the difference between Central Alveolar Hypoventilation Syndrome and Sleep Hypoventilation Syndrome, if any? One comes from the ICSD and the other from the standards and practices section of our looseleaf.

2) Why is end tidal CO2 endorsed by the American Thoracic Society guidelines for pediatric sleep studies, but rated "D" level evidence in Standards and Practices (the worst)?
