1) What is the correct management when arousals due to GERD is in the differential? Does one do a PSG with pH probe? Or does one refer for 24 hour pH monitoring? Kreiger suggests the latter, but does not explain how other disorders would be ruled out.

2) What are the situations in which one would "wake pt up?" Sinus pauses of how long and what frequency? Ventricular tachycardia of what length or frequency?
