Mallampati began, I believe, with anesthesiology in an attempt to predict difficult intubations. It was modified in the ENT literature as a clinical attempt to predict obstructive sleep apnea. See Friedman-Clinical Predictors of Obstructive Sleep Apnea in Laryngoscope vol 109; Dec 1999 page1903(on or about that page)
