paradoxical mentions in ICSD and choking question

There are unexpected characteristics mentioned in ICSD that I would like to list and invite comment on:
1) idiopathic insomnia: reversal of first-night effect (I thought this associated with psychophysiological)
2) jet leg: increased awakenings to urinate (I thought this part of OSAS)
3) bruxism: scalloped tongue (I was taught this is indication of macroglossia)
4) nocturnal leg cramps: relieved by movement (obviously, usually part of RLS)
5) asthma: paradoxical respirations (not from ICSD, but suggested in Shepard atlas, and I associate this with OSAS)

Finally, I would like to know how one would differentiate diencephalic szs from sleep-related choking or GERD.
