A 45 yr old man with CMT complained of orthopnea and exertional breathlessness. He also stated that he became severely short of breath when his son lay on his stomach as early as 8 years ago. No snoring, sleep occasionally unrestful. No medications.
On exam some distal wasting, normal pharynx,abnormal gait and abdominal paradox when supine.
Oh yeah, he also said that he recently punched his wife one night. His brother was tawnting (sp?) him through a window with a cigarette and he tried to punch him!
How would you manage this patient?
