Ten secs was chosen because the typical length of a breath in adults is 5 seconds so an apnea would be no airflow for 10 seconds, or two breaths. You should be able to use the two breath run in children who are breathing at a faster rate. So on an exam if I were to see 8 second apneas with a child who is breathing at 4 seconds per breath, I would count it. However, I haven't read the ICSD, just information I learned from lectures.
