15 month olds sleeping problems

My wife and I can't seem to get our 15 month old to sleep thru the night. My wife is still nursing him, but she is tapering that off. He wakes up every night about 2 and my wife will nurse him and he used to fall right back to sleep. Now he stays up crying. We recieved a brochure from his doctor about helping us to train him to sleep. However it doesn't seem to be getting better only worse. My wife will go in his room every 5 minutes just to lay him down and pat his back (per brochure). He will lay still and seem to be sleeping. She will leave the room after a minute and before she makes it to our room he is standing back up and crying. We wait 5 minutes and repeat the process. He takes two naps a day usually from 9:30 - 10:30 and 2:00 - 3:30. Sometimes these times are longer sometimes shorter. I am a tired and still trying to be understanding husband, but my wife really needs some rest. Are there any suggestions?
