From my personal experience, I found that my baby responded to sleep training and fell into a much better sleeping schedule once I set her on an eating schedule. Maybe nursing on demand at this point is keeping him a "grazer"? It sounds like he uses nursing to comfort him and hasn't learned to comfort himself to go to sleep. I really believe that for most babies falling asleep and getting themselves to fall back asleep if awakened in the middle of the night is a learned skill, not just something some babies pick up and others don't.
