10 wk old-no crying at night yet seems distressed

My 10 week old son has never cried at night. He does not, however, appear to be having a peaceful sleep. He spends the night thrashing around in his cradle, flailing his arms, and rolling his head back and forth. He grunts and snorts, sucks his fists, and sometimes has his eyes open. He seems very uncomfortable and yet he does not cry. Often he settles on his own but not for very long (a few minutes) before starting up again. The doctor has repeatedly said that if my son needs me he will cry but I'm still waiting to hear it. Often I get him up to feed him once in the night but the remainder of the night keeps to the same unsettled pattern. We've tried having our son in bed with us but nothing changes (except we get no sleep!). I know it's a funny thing to be worried about but if he is hungry or hurting I want to know he'll cry to let me know. Sometimes if he goes through the night without a feeding, he doesn't even cry in the morning when I know he has to be hungry.

A few extra notes: I am breastfeeding; my baby doesn't nap well at all during the day and when he does nap he wakes shortly after often with painful sounding cries; it is no small task to get him to sleep day or night; the doctor believes he has reflux therefore we are doing all things possible (except medication) to combat this problem.
