13 week only naps 45 minutes at a time even though he is exhausted

My son Simon is 13 weeks old. We follow the Eat/Activity/Sleep schedule. I look for cues that he is tired yawning, etc. When he yawns we go to the nursery for quiet time. Rocking, singing and/or reading. When his eyes get droopy I put him in the crib. He may cry for 1 to 3 mintutes then he is out. He sleeps for exactly 45 minutes and will not go back to sleep even though I can tell he is exhausted. I will pat him and reassure him that I am there and he crys and crys at full strength. I can get him to go back to sleep if I rock him into a deep sleep, but I can not repeat the way I got him to sleep by getting him drowsy by rocking and then putting him in the crib...he just cries even harder. I am at a loss. I know he is tired!!! PS. He goes down well at night and sleeps for 5 to 6 hour stretches. Any help would be appreciated.
