Ambien Survey. ***PLEASE HELP!***

If you have ever taken Ambien, recreationally or by prescription, please help me out by filling in the following information and emailing it to me. I am working on a report about Ambien and could REALLY use this information. All information is strictly anonymous and will not be used for any other purpose than the report I am writing.

_How long did you take your presciption?
_Did you finish your prescription?
__If not, what was the reason for discontinuing use?
_Did Ambien work for what it was prescribed for?
_Did you ever have side effects that are not listed as common side effects for Ambien (note 1)?
__If so, what were they?
__If so, were visual/auditory hallucinations one of them?
_Were you on any other medications at the same time as Ambien?
__If so, what medications?

If you have ever taken Ambien recreationally, that is, for any purpose other than as a sleep-aid:
_What was your reason for recreational use?
_What effects were you looking for?
_Did you get the desired effects?
_Would you use Ambien again recreationally?
_What dosages (or average dose) did you take?
_Did you ever combine Ambien with other medications or recreational drugs?
