AJM, I sympathize with you. I too have tried everything. Sleeping pills don't work - although I am now up to an average of about 4 1/2 hours of sleep with Neurontin (an anti-convulsant - not really a sleeping pill).

That is still not nearly enough sleep - not when I lecture for 5 hours straight the next day in my back to back classes. But it is a lot better than the 2, 1, or 0 hours I used to get!

I too have had a good exercise routine. I too have tried to use excellent sleep hygiene. I have tried SR and conditioning. It just doesn't work for me. It seems nothing does, and I really struggle with that. Sleep seems like it should be such a natural and easy thing to get, and things that are called "sleeping pills" should cause sleep!! Afterall, that is what they are called.

I guess this is why insomnia is so hard to treat, because of the huge differences in each person's situation.

I've been told by doctors to get up every 15 to 20 minutes if I am not asleep yet and to go back to bed when I feel tired enough to fall asleep. If I did that I would NEVER sleep at all - really and truly never sleep AT ALL. I have NEVER been able to fall asleep in under 2 hours. If I get up, the two hours starts all over again. It has nothing to do with not being tired enough for me either.

I am sometimes too tired to roll over and am very relaxed - not anxious at all - etc, but cannot cross over into sleep. - VERY FRUSTRATING!!

At least my "diagnosis" helps me gain a little peace. At least a doctor has labeled me an idiopathic insomniac which means they cannot find an underlying psychiatric or physical cause (e.g. apnea, restless leg, depression, anxiety, poor sleep hygeine). At least my doctor is willing to say it is a primary insomnia - a problem of its own not caused by anything else. Of course, this does not help me get relief, but it is nice to have a doctor support what I already knew to be true.

I am really glad that SR has worked so well for many people on this site. I WISH it worked for me. I have tried it and been very meticulous about doing it well, but it doesn't help.
