There are many things that you can do to improve your sleep. First of all, you are sleeping irregular hours. Sleeping 4 hours one night and 12 the next (which is way to much), will really screw up your circadian rhythms, making it very hard to fall asleep the next night. You must go to bed and get up at the same time everyday, but only go to bed when you are sleepy, not tired. There is a differance. It is alright to go to bed later than usual, but not earlier, even if you are sleepy. You must try and stay awake. A regular schedule is one of the most powerful things you can do for your sleep.

Another thing, is caffeine can stay in your system for up to twelve hours, so I would definately cut out the caffeine eariler than two hours before bed. I would also like to suggest that you try and give up smoking. It can cause insomnia and is bad in everyway, so do it for your health. I have alreay lost three relatives to lung cancer. Cigarettes kill far more people than alcohol does.

If you have a hard time turning your mind off before going to bed, I have heard that the book "Say Goodnight To Insomnia", is excellant in helping with anxious thoughts associated with the fear of not sleeping.
