I would recommend tapering off the ativan and using it only to break a bad streak or for important events. It was good advice to do a less strict form of sleep restriction. Go to bed after a certain time but only when you are tired and stick to the same wake up time. Even if SR does not work quickly, it gets easier. Insomnia is brutual. Waking up in the middle of the night is just another type and or phase of insomnia. Lower the arousal, create the sleep debt, and anchor the sleep. If you are doing this, and it is not working, do it more. Sometimes it is a marathon and not a sprint. For me, it took 18 months to get significant progress. For others, just weeks.
