I think your situation closely matches mine, right down to having negative thoughts while trying to distract myself.
One of the things that has helped my onset problems is that I catch myself getting tired in the two or three hours before bed. Once I was very tired, but then later got scared that I wouldn't be able to get to sleep and then couldn't. However, I turned this into a positive experience because it showed me that there's nothing physically wrong with me, my body wants to sleep, it's just my interference with that by worrying that keeps me up. I've reinforced this fact enough that it's not much of a problem anymore, so it will likely be so with this thought pattern eventually too.
The clock thing is true, too. I've actually woken up about two or three hours after getting to sleep almost every night for a month or longer, and most of the time I can easily get back to sleep...except for when I look at the clock (I usually put a piece of cardboard or a letter in front of it).
Something that's kind-of funny is that I've devised a method for logging when I wake up that doesn't require me to look at the clock. I've taken my PDA and put a piece of cardboard in front of it at night with certain sections cut out. I've got an application which will allow me to put a timestamp on a notepad, and I can operate the appliication with the cardboard over the front of the PDA without seeing the time! I slipped and accidentally saw it though last night, which was bad.
