This was almost exactly my pattern. Some nights I could fall back asleep around 4-5, sometimes not. I used a combination of Remeron and cognitive- behavioral therapy (for stress, anxiety and worrying about sleep) to get to a point where I was sleeping around 6 hours every night,in bits and pieces, but I couldn't get the whole night back.
I finally decided to quit my stressful job and work part-time while starting a private practice. Best decision I made. I left a month ago and my sleep has improved dramatically. I know now that stress and worrying were very much affecting my sleep. I thought I could handle the job pressure but decided it wasn't worth it, and I made the right choice.
Not telling you to quit your job (unless you determine as I did that it is the main obstacle to your recovery)- just pointing out the various things that helped me to recover.
