I also experienced horrific insomnia and anxiety, to the point where I could not work or drive. But I decided to push on and no matter how bad I felt, I'd get up and go to work and do all the things I used to do. It was very, very hard - especially at first. Slowly but surely I got better. And all along I kept telling myself "what is the worst thing that will happen to me if I don't sleep?" Now, 2 years later, I am more or less cured. And if I can't get to sleep I just tell myself, "who cares, I'll just be tired tomorrow." This approach takes the pressure off and allows me to relax. I think the main reason I got insomnia is because I was so dang worried all the time about NOT getting sleep. My anxiety was self-induced. Insomnia lead to anxiety, which lead to more insomnia, which lead to more anxiety. It was a vicious circle. I broke the circle by taking the anxiety out of the loop. It was not easy and it took nearly 2 years, but it worked for me.
