I think we basically agree. Underlying issues need to be addressed, and CBT is a very helpful tool in that.
Where we disagree is whether or not there can be an "underlying cause" that isn't psychological but is linked specifically to sleep. I wouldn't have believed that either until recently, after I addressed all my sleep anxiety, decreased my stress and then stopped the Remeron and found I couldn't sleep. I lay in bed, night after night, awake as could be, and not the least bit anxious, and then when I took a smidgen of Remeron, my sleep cycle went to normal.
I do have anxiety but I don't think that's what's keeping me awake, unless by being anxious for 40 years I've gotten myself into a state of arousal that just can't get lowered without medication. I chose to use the metaphor of "broken sleep system" to describe my theory. The fact that Remeron in half the normal dose, a drug that doesn't target anxiety, helps me, leads me to believe that it isn't just anxiety keeping me awake.
Truth is, all we both have are theories,and as I said, until I see some evidence saying it's not possible, I will chose to believe that you can just have a sleep system that doesn't work right.
