Have you tried the sedating antidepressants? I would urge you to get a second opinion, or even a third or fourth. Most doctors (even so-called "sleep experts") don't really know everything about the causes of insomnia, and having "burned out receptors" sounds fishy.
I believe that I also have an organic reason for sleep issues (don't know what it is, I think it's related to family history of mood disorders) and CBT helps only in that it takes away the frantic worry about insomnia that can make it worse. I have had good success with first Amitriptyline and now, Remeron.
