One thing I didn't see on your list is calcium/magnesium (liquid) supplements. Not that that's going to be the magic bullet after all you've tried, but still.

Re: homeopathic remedies. Calms Forte is a generic homeopathic remedy, and I'm guessing quietude is also. Have you seen a naturopath to have a specific homeopathic remedy prescribed for your set of symptoms? There are so many remedies, and they are very specific to your personality type, health problems, etc. I've had a good response to this approach.
