My long-term insomnia...help please

I have insomnia for almost 15 years now. The phase (average of 3 nights) comes and goes. I get the insomnia phase around 2 to 3 times a month. It would be good if I have gotten used to it but whenever it happens, it seems like something new. New fear, new anxiety, new depression and new frustration! I have both the hard to sleep or the early awakening type. But as far as I can remember, I hardly have both at the same phase. Just like a lot of insomniac, my main enemy actually is FEAR and ANXIETY of being able to sleep or sleep back. This is compounded by my urination in the middle of the night. No matter how much I try drinking liquids near bedtime, I am still awaken by my urge to urinate.

Can I humbly request valuable tips on killing anxiety, fear and urination at night? Thanks!
