Hi Wolverine,
At this point, I am definitely a maintanence insomniac.
I have developed onset issues off and on, especially when I went to bed very early to try to "catch up" on sleep. The one thing SR works on VERY well for me is the onset problem. SR will clear up these onset issues in about 2 days, but the maintanence problems are much harder to take care of.

My symptoms at this point are:

I fall asleep within 5-10 minutes of turning off the light
I wake up after 2-4 hours of deep sleep
I have trouble falling back asleep and if I am able to go back to sleep, generally it takes about 30 min-1.5 hours. I can fall back asleep (eventually) about 70% of the time.
My sleep is deep at the beginning of the night, generally much lighter as the night goes on.
Very few sleepless nights
Unable to sleep more than 6 hours in a night-range between 3-6 hours.

Remeron worked very well with maintenance issues for me. While taking the medication, when I woke up (I rarely would sleep through the night), I could fall back to sleep very quickly.
I have read the treatment is generally the same for onset and maintenance issues because it is believed they are both related to anxiety. With sleep maintenance, in the beginning of the night, the initial sleep drive overcomes the anxiety and allows small amounts of sleep. As the night wears on and the sleep drive diminishes, the sleep gets lighter, the anxiety breaks through and we wake up. We find it hard/impossible to get back to sleep. I don't necessarily agree with this, I tend not to feel particularly anxious when I wake up in the middle of the night. I do get frustrated, but not anxious.

Very interesting topic.
