Sleep disorder clinics

Can anyone recommend a sleep disorder clincic, anywhere in the world? I am living and working in Japan and my office have agreed to let me have an extended sick leave to try and address this problem. Living in Japan means I cannot consutl an English speaking physician for a referal. I am probably a pretty typical insomniac, dependant on medication which has slowly lost its effectivenss, but when I read the messages from people who seem to have an identical problem, yet have had to endure it for years and years, it terrifies me. I have had only to endure insomnia for a year and I feel like I can't endure it for much longer. Please can anyone help.
