Have you tried doxepin, a sister drug of amitriptyline (which also made me feel stoned but denied me sleep)? It works for most people. You may have to go to 75 or 100 mg. Main problems are constipation, weight gain and lack of energy. I also found this drug hard to discontinue--no physical withdrawal, but no sleep for days on end after discontinuation. But then I found that with all the drugs I tried.
