Provigil Cost & Insurance Coverage

Just wondering if provigil users are going through their insurance companies to obtain this drug or buying it on their own. Just checked a list of "approved" drugs on an insurance pharmacy list that the company that I work for has supplied us & saw that it is not listed. I heard that this is a very expense drug. How have your experiences been with obtaining/purchasing this new drug? My daughter may be prescribed this med sometime soon.
