Usually a person works their way up on the doses.

Earlier this year I was on 200 for 1 week, 400 for next week, then 600 and then 800 and I felt nothing. I stopped because of surgery and now back to 200 am and another 200 lunchtime. I doubt I willl go up to 800.

It was suggested by a doctor because 400 quit working but then I have excessive daytime sleepiness and it was getting worse.
