My understanding is that ADHD and Narcolepsy, though both neurological disorders, stem from different causes. They certainly share some symptoms. For example, if a person has narcolepsy with daytime sleepiness, they will be less focussed and less able to concentrate. And, many report being help by taking stimulant medications. Also, various sleep disorders cause daytime sleepiness, which depending upon the severity, may then be diagnosed as ADHD or Narcolepsy, even when neither disorder turns out to be present. Caffiene most often impacts sleep patterns and quality of sleep, even when we believe we can drink 20 cups of real coffee every day all day and sleep like a baby. So, the problem of daytime sleepiness, depending upon severity, can often warrent further assessment. Diagnostic sleep labs testing can be an important diagnostic tool, often nailing down clear specific diagnosis and treatment. And, yes, some as simple as gradually removing caffiene from the diet.
