Been There

I haven't been there near as long as you. Course I am no where near as old as you. I am a mere 51, finally dx'd at 41. I was an athlete, not a musician, but still drank more than a few tequila's or what ever else was around. I am also a recovering alcoholic. I can now trace the onset to pre teen years if the vivid nightmares are the indicator. Otherwise it would be about 17 or 18 when I started to have recurring "zingers", which were then put down as a football thing.

So, by 17 or 18, I started to show C, and it was what finally drove me from baseball. By then I was drinking regularly and smoking, too. My parents never smoked in the house or in our presence as children, any damage done, I did it. I had never heard of that link. Frankly, my lingering suspicion is head trauma. Mostly self induced, too.

I don't know if this helps you, expecially in light of SK's attidtude and tolerance of smoke. I too, live my life fully...though I've never gotten my pilots liscense. Good for you. Let us know if you figure out a link, ok?
