Yeah, that is exactly like I was when I was taking 400 mg a day, back when someone came up with the phrase "the provigil 'stoopids.'" This is a good example of what I think of as the stoopids gone even further awry. A little bit too much provigil over a long period of time, and you get stoopid. A lot too much provigil over a long period of time and you get drunk/stupid/goofy/panicky/off balance. (This is not, of course, a medical description, but something that I think will at least make sense to those of us who have had the stoopids. )
I felt exactly like you just described for a good week or so on 400mg before I called my Dr, and it got to the point where I would dry heave when the pill touched my toungue in the morning, much in the same way a whiff of tequilla could make your stomach turn after a night of partying. I think it was my body saying "hey hey...what do you think you are doing to me!!"
I was afraid of everything, and normally, I am pretty fearless. While looking at a children's book about spiders, I literally had a freak attack everytime my fingers would come in contact with a creature on the page (and I studied entomology for a while...african hissing cockroaches could sit in my hand no problem). Plus, I was falling all over the place, walking into walls, etc., and I completely lost my personality...I was like a shell of a person.
Basically, the only thing that has made me better was quitting the provigil and giving my brain several months to readjust. Now I can take smaller doses (25-50mg) that just take the edge off of sleepiness, but I am even afraid to do that every day (so I don't).
I could literally go on for pages and pages...I will stop here...but I recommend you do a search or look on the forums from last june-august, where i know at least I was posting constantly with some new werido symptom from stupid provigil.
Best of luck, and listen to what your body tells you...even in the face of what your physician might say. weigh both sides, but only you can know exactly what you are feeling.
