That's a good topic! It's not for nothing that sometimes people go a long way to get their diagnosis (no matter which one)! There are always so many things that have to be ruled out! Your story shows that it's important not only to focus on N when somebody complains about sleepiness but also to rule out other things. I think most of the times it's the other way round with somebody getting tested for nearly everything else and then finally being diagnosed with N after a too long time of suffering. There are many diseases that are much more common than N. But it's easy to forget about the others when having one.
The problem is: You can have more than one diseases at one time and there are a lot of disturbances that can cause tiredness as a main symptom or side-effect.
I thought I add a list of the most common ones: thyroid problems or other hormonal disorders, diabetes, chronic infections, allergies, renal problems, liver diseases, gluten sensitivity, anemia and low blood-iron levels, malign diseases, multiple sclerosis, borreliosis, medication and many more. Most of them have additional symptoms and most of them show up in the routine investigations so what should be done additionally to rule out other things when diagnosing somebody with N is, besides a thorough history-taking and examination, a routine lab with tests for inflammatory activity, thyroid, liver and renal function, maybe a testing for borreliosis, a MRT-scan and, if there's any hint on something else additional specific investigations. Detecting something else doesn't mean the former dx of N was wrong. It's possible to have lice and fleas at the same time, BUT if two things make you tired fighting both will help much more than fighting just one!
