I take 54 mg concerta, 400 mg provigil, 10 mgs ritalin average 2 times a day. My insurance company has covered everything and in fact the provigil and concerta is mailed to me from their mail order pharmacy. You might have to get a preauthorization - they made my doctor fill out some forms re: provigil before they would pay for it. It doesn't sound like you have a "pre-existing condition" issue or they wouldn't cover anything at all. I don't think the insurance company can tell you what dose to take if the doctor is prescribing it. They sound like they are jackin you around. Good luck
