There are times where you have to try to stay awake.
There are also times when you are better off managing a "schedule" during the day when you can rest. After all, we really do need naps during the day.

Without your medications, this is a time to rely heavily on a rest schedule. Understand you'll have two, three or four more times during the day that you'll get drowsy.

I think you should tell your boss what you have told us: Your sleep specialist suspects you have narcolepsy and wants a sleep study. He requires that you have a one-month period before the test without medication.

Have your boss understand that your job performance will obviously be affected. It is your boss's job to understand your needs as an employee; and he has to provide some flexibility for a medical condition such as this.

Before talking with your boss, you should talk to someone in human resources. This is one of those cases where an HR verdict is something a boss needs to abide.
