In the case of neuronal stem cell therapy they take a few special cells that have not made a permanent descision to be a liver cell or kideney cell etc and fuse them with cancer cells ,then after the cells have multiplied as much as they want they add a lot of vitamin A so they won't be cancerous anymore. Then they inject the cells into the watery spinal fluid part of the brain and the stem cells migrate to wherever and replace any cells that are not there but should be. I don't know how they measure orexin in spinal fluid but they have a number of standard ways of doing that type of thing like which invole a type of tageing , for instance radioactively labled antibodies (which the orexin would bind to) or similarly a molecole is attached that shows up under black light
