Since I found out that narcolepsy prevents much time in deep sleep stages, I have often thought that had factor in my neck problems. I have had similar problems to the way you describe with muscle spasms in my neck. I have heard that it is during deep sleep stages that the body repairs tiny muscles tears, and that sleeping well is very important to recover from back and neck injuries.

I think that cataplexy is often the cause of strain on my neck. My muscles are generally pretty tight, and seems to me that suddenly having my head slack can't be good for those tight muscles. Also, my neck problems started in my late teens, along with cataplexy. Once I was being chased by my friends (kidding around) and fell really hard from an attack of cataplexy. My friends yanked me up while is was lying on the ground. I was unable to move or speak to tell them to not touch me. I'm not sure if it was them yanking me up or the fall itself is what hurt my neck, but I was taken to the hospital because my neck was it such a spasm that my head tilted so far it was practically touching one of my shoulders. I stayed in the hospital for two nights, and they ran a MRI, a cat scan, x-rays, etc., none of which showed any problem with my neck or back. My neck got better from the muscle relaxants. It was very scary at the time because I thought the paralysis was from the neck injury.
