I have often worried about a scenario like yours. I have never had an accident due to narcolepsy, but I have had some close calls (pretty infrequently though). The funny thing is that sometimes I can just drive forever and not get tired at all -- plus I'm a really good driver. I drove cross-country with a friend a few years ago and I was so awake with very little meds. I trust that you are a good driver too -- get another doctor and fight for it if you have to!

P.S. I find that driving a stick-shift keeps me more alert than driving an automatic. Having to do something rather than just sitting there keeps you on top of things, and driving a stick-shift is more fun anyway.
