reading your post, it sounds so much like the stages i went through, and although i think you've already been to your doc. i thought i'd still share.
the thing that caught my attention is that you take effexor xr and its starting to wear off. i worked my way up to 300 mg, and then went to my psych. desperate when it was no longer working enough. instead of increasing that more, we decided to keep it the same, but add welbutrin. he also knew i took ritalin for narc. and said welbutrin is sometimes used to increase the effects of it, so it was like a 2 for 1 deal. :)
i believe that was a little over a year ago and as far as the depression goes, the med combo has worked great. i have not had to increase the meds since and have not had any major depression 'break downs' (when i can do nothing but lay and cry, closing myself off from the world).
the effectiveness of the ritalin is a little off and on. when regular ritalin stopped working as well, i gave provigil a try, which did nothing at all for me, so i changed to ritalin sr, twice a day. i still get some sleepy spells at work now (i didn't at first), but not as bad as before, and i'm not falling asleep at the wheel.
anyways, just wanted to share how well the addition of wellbutrin to my daily meds has helped me since your reaction to different meds seems so similar to mine.
