You mentioned taking the trach out for awhile at night. Please be sure to follow your doctor's instructions as to the care of your trach. After a few years, I got to the point where I could take my Montgomery out and not worry about my trach closing down. Longest I have gone without any tube at all is about 18 hours. Keep in mind that that was after I had my trach for six or seven years. And, I don't make a habit of it. I do remove my Montgomery tube entirely twice a day to clean it thoroughly, but I don't usually leave it out for extended periods. Gets to be kind of like wearing a watch: you only notice its absence.
As to getting comfortable with your head and neck, I use a soft pillow and put it mostly under my neck to help keep my neck higher and head a bit lower.
