To find the dental appliance....

I'm fairly certain I have mild apnea-- not small enough to ignore and not large enough for a cpap. I pretty much know that a dental appliance is the way to go.
I have the sleep studies (that show I have mild apnea) but no dental insurance. I don't want to pay some dentist out of pocket for him to sell me some dental appliance I could just buy.
So-- where could I just buy the appliance? Does it have to be fitted or custom made? Do I need a dentist's perscription? Thanks-- John.
